Responsibilities:

• Conduct assessments to identify clients' needs and develop personalized service plans.
• Connect clients with community resources and services such as healthcare, housing, and employment.
• Provide counseling and emotional support to individuals and families.
• Maintain detailed case records and monitor clients' progress.
• Advocate for clients' rights and assist in crisis intervention situations.

Requirements:

• Bachelor’s degree in social work from an accredited school of social work
• Three years of recent case management experience
• Two years geriatric experience
• Recent Physical and TB tests (within one year)
• Driver's License or State ID
• Willing to travel 20-30 miles radius
