EverStrong System Navigator

Supervised by:EverStrong Program Director

Hours: 40 hours per week, must include EverStrong Drop-In Center hours which includes at least two Saturdays per month.

Classification: Non-exempt

Position Summary: The System Navigator plays a key role in providing intensive, housing-focused system navigation services to program participants to support them through all stages of their journey to housing stability, with the ultimate goal of supporting the young person to thrive socially, emotionally and holistically! The System Navigator provides mobile engagement, support, and case management for unaccompanied youth experiencing homelessness or housing crisis, one-on-one community resource navigation, support in creating individualized goal plans, and assistance in helping reach those goals. The purpose of the System Navigator is to help homeless and at-risk youth increase independent living skills and prevent them from entering the homelessness services system if possible or help them to rapidly exit the homeless system into the safest and most permanent arrangement available.

Position Responsibilities and Essential Duties:

 

System Navigation Responsibilities and Duties:

  • Assess housing strengths and barriers of program participants experiencing homelessness to determine housing and service needs, and provide support in assessed areas

  • Meet regularly with program participants to provide intensive, person-centered system navigation, case management, and service coordination services to ensure clients have access and receive services necessary for housing stabilization.

  • Work closely with clients to address housing barriers, develop short and long-term goals related to housing, employment and other self-identified needs, and provide support and resources to empower participants to reach goals.

  • Facilitate transportation of program participants by booking ride share services, coordinating with other agencies, or providing transportation directly with other staff present.

  • Assist clients with gathering documentation, securing benefits, identifying appropriate and affordable housing options, and securing housing of their choice.

  • Serve as on-going liaison between program participants and property owners/managers, including providing mediation and advocacy on the participant’s behalf to obtain/maintain housing and educating program participants of their rights and responsibilities as tenants.

  • Respond to community referrals for intervention and support and conduct frequent outreach to connect and build rapport with individuals experiencing homelessness.

  • Collaborate with other EverStrong staff and community service providers to facilitate referrals to and from community organizations, and provide information and assistance regarding available support from social service agencies and/or community programs.

Other Responsibilities and Duties:

  • Assist with staffing and operations of the EverStrong Drop-In Center.

  • Work alongside other OutReach staff to create and implement programming to support the LGBTQ+ community, including organizing special events, facilitating social and support groups, and leading LGBTQ+ cultural humility trainings as needed.

  • Attend weekly and monthly meetings with OutReach and EverStrong staff as required by supervisors

  • Network with other services providers to share information and resources.

  • Record client interactions, including maintaining case notes and records of participant services using Homeless Management Information System (HMIS), in a timely fashion to maintain compliance with agency and program policies and procedures.

  • Communicate with supervisors and other staff about the needs, skills, and goals of peers.

  • Attend positive youth development, trauma-informed care, supervision, and other trainings as required by supervisors.

  • Identify opportunities and attend conferences, meetings, or resource fairs to share information and resources about the LGBTQ+ community, especially LGBTQ+ youth/young adults experiencing homelessness or housing insecurity.

  • Assist in the planning and day-of coordination of special events and programs organized by OutReach such as the Magic Pride Festival.

  • Additional duties as required by supervisors

 Qualifications and Requirements

  • Professional experience in supporting people who are unhoused or dealing with housing insecurity and / or lived experience of housing insecurity.

  • Professional and / or Lived experience supporting people to overcome barriers in their life, including peer mentorship, crisis support and trauma informed care.

  • Must have and maintain a valid state driver’s license, maintain automobile insurance coverage and have access to an automobile. Will be required to use a personal vehicle to transport clients at times.

  • Understanding of trauma-informed care, harm reduction, and the housing first model.

  • Excellent written and verbal communication skills, including experience with public speaking and community outreach and engagement.

  • Experience engaging with a diverse population, including youth and young adults, people of color and LGBTQ+ people, and strong interpersonal skills with an emphasis on professional and personal boundaries and mindfulness of power dynamics and personal biases.

  • Computer experience, including proficiency in Microsoft Office suite and Microsoft 365.

  • Candidates with lived experience of homelessness will be prioritized.

  • Bilingual (English/Spanish/Hmong) candidates will be prioritized.

Travel:

Travel is primarily local (within the City of Madison and Dane County).

Physical Demand/Work Environment:

Physical Demands: The physical demands and work environment described here are representative to those an employee encounters while performing the essential functions of this job. While performing the duties of this job the employee is regularly required to use hand, reach with hands and arms, talk and hear. The employee will also walk, sit, stoop, kneel, crouch and lift and/or move up to 30 pounds. Vision requirements include close vision, distant vision, peripheral vision, depth perception and ability to focus.

Work Environment:This employee will work in a professional office environment, which requires use of standard office equipment like computers, phones, copiers/printers, and filing cabinets. This employee may also work outside in variable weather conditions as part of community outreach activities and be exposed to a variety of conditions, including light and noise levels, at client sites.

Hours:This is a full-time position and core hours and days must occur during OutReach and EverStrong Drop-In Center hours, which includes occasional weekends and evenings as needed.

Compensation: $22-$24/hr, eligible for benefits. OutReach pays 100% of benefits for full-time employees. Ten paid vacation days, sick time, and personal time.
