Duties and responsibilities

The incumbent, an employee of the Florida Department of Health Putnam County, works to protect, promote, and improve the health of all people in Florida through integrated state, county, and community efforts. The incumbent is expected to promote innovation by searching for creative solutions and managing resources wisely. They are expected to be accountable and perform with integrity and respect. This is a professional Social Services position that serves as the Family Support Worker and is responsible for assessing and providing for the delivery of social services to individuals referred to the Healthy Families Program. The incumbent is also responsible for providing in-home support to families receiving services through Healthy Families Putnam. The work will be performed under the direct supervision of the Human Services Counselor Supervisor and Human Services Program Specialist. The primary responsibilities of the Family Support Worker are as follows: The Family Support Worker will provide intensive home visits to families that volunteer for Healthy Families Services. During the home visit the FSW will model appropriate parent-child relationships, enhance the parents’ abilities to create a stable and nurturing home environment, and help the parents to provide children with experiences that stimulate healthy brain development. The incumbent will also demonstrate knowledge and skills with topics such as: child development, discipline, dynamics of child abuse and neglect, and demonstrates the ability to maintain and update resource information. Conducts initial engagement efforts to potential participants, to include; phone calls, emails, snail mail, drive-by’s. Assists in planning and the facilitation of participant events and community outreach events. Coordinates activities designed to create and maintain favorable public image to assist in raising the awareness for the Healthy Families program. Promotes the Healthy Families Putnam program to local businesses, community partners/agencies. Assists in distributing letters, flyers, brochures or other informational/educational documents to inform the community about Healthy Families Putnam program and offered services. Assists in soliciting In-Kind donations and assist in obtaining required items for new Fiscal Years. Assists families with community resources that parents build upon to strengthen their support system and assist with achieving their family support plan goals and leads to greater self-sufficiency. Encourages parents/caregivers to ensure that well-baby visits are received in a timely manner. Promotes child health and development and help ensure the families’ social and medical needs are met by linking the parents and children to a medical home. Conducts assessments using Healthy Families Florida Assessment Tool. Correctly and accurately documents narrative, score sheet, and intake information. Provides information referrals, and educational materials to all families interviewed upon assessing their needs; documents referrals and assistance offered on individual charts. Compiles and enters all screen and assessment data into the Healthy Families Florida database as required by HFF. Participates in required program training provided by the grantor and consultants and is willing to provide backup family support services during periods of insufficient staffing such as turnover, illness, or other periods of staff absence. Assists in training new staff on all aspects of this position. Participates in individual supervision weekly for no less than 1.5 hours. Maintains and updates client records by entering their own data into the Healthy Families Florida Tracking System. Scans all documents into a digital file for each participant. Runs reports in the Healthy Families Florida Performance Management System (i.e. Due Dates, Outcomes/Process Measures, & Missing Data) periodically to ensure that all tools are administered within due date windows, home visit data is accurate, and all missing data is obtained & submitted. Reports all inaccuracies to the data entry person so that corrections can be made. Provides developmental screenings to help detect potential delays and make referrals to appropriate services to address suspected delays. Conducts home safety assessments and provides families with appropriate safety tip sheets within the first year of the child’s life and as needed thereafter. Conducts home safety observations, according to Healthy Families’ policy & procedure. Responsible for scheduling and meeting with families in accordance with the families assigned level as instructed in the current Healthy Families’ Policies & Procedures. Conducts weekly outreach efforts within local OBGYN offices and weekly group sessions within the Putnam County Jail. Participates in staff meetings, department meetings, team days and community events, as appropriate. Participates in community meetings monthly, as appropriate. Completes timesheets, EARS, and leave requests accurately and promptly. Performs other duties as assigned or required.

 

Required Knowledge, Skills, and Abilities:

Knowledge of basic counseling techniques and practices. Knowledge of the basic skills required to teach, recognize, and understand parents, infants, and children, as well as the differences between functional and dysfunctional families. Ability to establish and maintain working relationships with others. Ability to recognize the basic signs of child abuse, neglect, exploitation, domestic violence, and substance abuse. Ability to be compassionate without being enabling. Possession of good oral and written communication skills. Experience working with or providing services to children and families, a willingness to work with culturally diverse populations that are among the target population, the ability to establish trusting relationships and accept individual differences, be knowledgeable about infant and child development. Possess basic computer skills and knowledge of basic computer programs.

 

Qualifications:

Minimum- High school diploma/GED and Willing to work with children and families in a home setting

Preferred- 1 year of experience working in a social services, health, or child development setting. Valid Drivers license.
