Position Overview
Are you passionate about delivering high-quality care and making a meaningful impact on patient lives? We're looking for a dedicated Registered Nurse (RN) Care Manager to join our primary care team. In this role, you'll provide comprehensive and coordinated nursing care to an assigned panel of patients, working closely with both VA and community services. Your focus will be on supporting health promotion, disease prevention, and managing acute, chronic, and long-term needs, particularly in Oncology and Hematology.
Contract Details
Location: This position is remote, supporting patients from the Durham, NC area.
Duration: 1-year contract with potential extension up to 2 years.
Schedule: Day Shift.

You can work remotely from anywhere within the 50 US states or the District of Columbia.
Key Responsibilities
Assess patients' health needs and develop or update plans of care to meet interdisciplinary goals.
Respond to significant changes in patients' health conditions promptly.
Provide direct patient care and health education to patients and their families, focusing on self-management, prevention, and wellness.
Coordinate patient admissions, transfers, and discharges.
Lead as a Patient Aligned Care Team (PACT) leader and serve as a mentor to colleagues or students.
Educate patients and families on health conditions, diagnoses, and medication administration.
Implement Care Coordination and Integrated Case Management (CC&ICM) for complex patients.
Offer comprehensive care for Women Veterans and engage in vaccination initiatives and other clinical services as needed.
Conduct accurate physical assessments, triage patient needs, and develop appropriate care plans for improved patient outcomes.
Manage your workload effectively and coordinate with community resources and programs to ensure seamless patient care across various settings.
Requirements
Bachelor of Science in Nursing (BSN).
Graduation from an accredited nursing program (NLNAC or CCNE).
Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications from the American Red Cross or American Heart Association.
Minimum of 3 years of experience in case management and in hematology/oncology.
Active and unrestricted RN license in any US state or commonwealth (state licensure for the state of practice may be required).
U.S. Citizenship required.
Proficiency in EMR/EHR systems and general technology.
Successful completion of federal background checks and credentialing policies.
Note
This position is contingent upon contract award, client approval, successful background investigation, and obtaining and maintaining a Public Trust / High Risk clearance.
