Position Overview
As a Registered Nurse (RN) Care Manager in Primary Care, you will play a crucial role in delivering comprehensive and coordinated nursing care to a designated group of patients. You will collaborate with both VA and community services to effectively address health promotion, disease prevention, and the acute, chronic, and long-term needs of our patients, all aligned with their goals and care plans. This position will primarily focus on Oncology and Hematology.
Contract Details
Location: This role will support patients through telehealth services from the Durham, NC Veteran Affairs Medical Center.
Contract Duration: Initial contract for 1 year, with the potential for an extension of up to 2 years total.
Schedule: Day Shift.
Remote Work Eligibility
This position is eligible for remote work as long as you reside within one of the 50 US states or the District of Columbia.
Key Responsibilities
Assess and identify the healthcare needs of patients, developing and maintaining individualized care plans to meet interdisciplinary goals.
Monitor and respond to significant changes in patient health conditions.
Coordinate care and provide direct patient care, along with health education focused on self-management, prevention, and wellness in alignment with patient goals.
Facilitate patient admissions, transfers, and discharges effectively.
Demonstrate leadership as a Patient Aligned Care Team (PACT) leader and mentor colleagues or students to foster professional growth and development.
Educate patients and their families on health conditions, medication administration, and care strategies.
Implement Care Coordination and Integrated Case Management (CC&ICM) for complex patient needs, including comprehensive care for Women Veterans and vaccination initiatives.
Conduct accurate physical assessments, triage patient needs, and make informed clinical judgments that lead to improved patient outcomes.
Organize and prioritize your work, collaborating with community resources and co-managed care providers to ensure seamless patient care across various settings, including non-VA care and specialty services.
Qualifications
Bachelor of Science in Nursing (BSN).
Graduation from an accredited college or university recognized by the National League for Nursing Accrediting Commission (NLNAC) or the Commission on Collegiate Nursing Education (CCNE).
Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certification from the American Red Cross or American Heart Association.
A minimum of 3 years of experience in case management and in hematology/oncology.
Current, full, active, and unrestricted nursing license in any U.S. state or territory. State licensure may be required based on the contract.
U.S. Citizenship is required.
Proficiency in electronic medical record (EMR/EHR) systems and general technology.
Successful completion of any required federal background check and VA credentialing policies.
Note: This position is contingent upon contract award, client approval, successful background investigation, and the ability to obtain and maintain a Public Trust/High Risk clearance.
