POSITION: SOCIAL WORKER ASSISTANT

RESPONSIBLE TO: Social Work Supervisor, Hong Fook Centers

COMMITMENT: 40 hours per week

STATUS: Non-Exempt (3 months orientation period)

JOB SUMMARY : Under the supervision of the Social Work Supervisor, the Social Worker Assistant will provide social work services to the participants of the Community Based Adult Service Centers.  This will include the intake of prospective participants, providing language assistance services, working with other team members to provide care management services to participants, and assisting the Social Work Supervisor in the psychosocial assessment of participants.

MAJOR DUTIES AND RESPONSIBILITIES :

• Introduces Hong Fook Community Based Adult Service Center programs to prospective participants and their family.
• Performs intake, home visit, and initial assessment of potential participants for services.
• Follows-up on the process of bringing participants in for program assessment, including gathering medical reports, scheduling transportation, and notifying team members.
• Engages in the enrollment of program participants, including an explanation of program rule and participant rights and assisting with new participants’ adjustment to the program.
• Provides care management services to the participants.
• Assists program participants to obtain needed services and benefits and acts as a liaison with outside agencies.
• Interprets various services or benefit notices and medical insurance statements to program participants or family members.
• Assists the Social Work Supervisor in the periodic assessment of program participants and maintenance of current written records in compliance with the licensing requirement.
• Assists in or helps conduct support groups for program participants.
• Participates in and helps organize outreach activities to the target frail, elderly, and disabled adult population.
• Participates as a member of the multidisciplinary team.

• Performs other appropriate tasks deemed necessary by the supervisor.

QUALIFICATIONS:

• BA in Social Work or related field
• Experienced and interested in working with frail elderly or the disabled population.
• Able to work in a diversified environment.
• Able to work independently and as a member of a multidisciplinary team.
• Good oral and written communication skills.
• Ability to make public presentations a plus.
• Bilingual in English and Cantonese/Mandarin highly preferred; ability to communicate in one of the targeted Asian languages (Vietnamese, Filipino, other Southeast Asian language, or Korean) desired.
• Flexibility in work shifts to meet program needs.
• Procession of a valid CA driver license and have the use of a reliable automobile desired.
• Ability to handle multiple tasks.

ADA REQUIREMENTS:

• Ability to communicate clearly with others.
• Ability to see and hear to interact with others.
• Ability to use a computer.
• To lift a minimum of 20 lbs.
• Vision 20/20 with correction.

SALARY RANGE: The target hourly rate for this job is $22.00 - $27.65 with benefits. The specific compensation for this role will be determined based on the education, experience, and skill set of the individual selected for this position
