Summary Under the supervision of the CD8 NC Program Manager, the Incumbent (System Navigator) will perform professional case management services and linkage for homeless consumers and assist consumers and their significant figures in understanding and developing solutions to problems that lead to and result from homelessness. The Incumbent will provide program coordination and system navigation activities in order to attach individuals to the Coordinated Entry System. S/he will assist in the coordination of pre-screenings, VI-SPDAT completions, scheduling assessments, referrals and linkages, record keeping, and other related work as required.
Essential Functions - Case Manager shall assist clients in obtaining additional services needed including but not limited to: identification, mainstream benefits, housing linkages (including crisis and bridge housing), substance abuse and mental health services, employment services, vocational services, education, legal services, credit counseling, money management, tenancy education, landlord relations, life skills training, family reunification, etc.
- Complete a CES survey packet with all homeless clients who have not previously been administered the survey packet or update the CES survey for those clients who have undergone significant life changes.
- Perform follow-up contact with clients in interim housing
- Collect data and enter information into information systems and HMIS
- Assist with addressing HMIS data errors if any
- Provide crisis intervention (Triage) and support including meals, hygiene, clothing crisis housing.
- Conduct pre-screening eligibility interviews and internal referrals as needed.
- Maintain communication and work with the building security team.
- Assist in preparing reports in accordance with funding requirements.
- Attend all mandatory program and division meetings and trainings, as assigned by his/her supervisor and division director.
- Conduct Problem Solving assessments and process Problem Solving requests for eligible clients.
- Assist with program outreach efforts to clients and the community.
- Assist with preparation for meetings and trainings and other group sessions as assigned by the Program Manager, Associate Director or Division Director
- Maintain appropriate boundaries; and adhere to SSG’s Code of Ethics and HOPICS Core Values.
- Represent the Agency in a professional manner at meetings and community events.
- Maintain files/records on client services in compliance with HIPAA, 42 CFR Part 2 and other funding requirements for audit purposes.
- Regular attendance required.

Secondary Functions Perform other duties as assigned by the Program Manager. Overtime, holiday, or weekend work may be required.
- Assist Clients in accessing three main Navigation Center services: Hygiene, laundry, and storage
- Assign and monitor shower usage
- Assign and monitor washer and dryer usage
- Pull storage bins, assist client in filling storage bins, audit the contents of storage bins, monitor and remove any perishable, toxic or dangerous items from storage, assist clients in accessing their storage
- Assist with maintenance and cleanliness of the hygiene, laundry and storage service areas
Minimum Qualifications - Knowledge, Skills and Abilities Required More than one year of case management experience. Basic knowledge of HMIS, Microsoft Word, Excel, and Internet. If in recovery, a minimum of three (3) years of being drug and alcohol free is required. Knowledge of basic filling structure, ability to organize and manage multiple priorities. Valid California Driver's License and Car Insurance. Ability to do concise documentation and effectively communicate in written and verbal form. Experience working with homeless families and the ability to work with clients from diverse cultural, ethnic, and socio-economic backgrounds. Attention to detail and punctuality are required, Bilingual English/Spanish required. Neat and professional appearance. TB test, CPR and First Aid certification required.
Supervisory Responsibilities This position does not have any supervisory responsibilities.
Environmental Conditions (Working Conditions) This position is responsible for work in “client friendly” environments, in the community, and some work in homeless encampments. This position will require extensive interaction with homeless individuals, individuals with severe physical and mental disabilities, and the possibility of individuals with infectious diseases. You may be exposed to aggressive/hostile clients. Incumbents must be ready to deal with traumas which may trigger personal trauma; must work in conjunction with other collaborative staff members; communicate with many sources including courts, Department of Children and Family Services, Law Enforcement, and other community agencies. There is some responsibility to work in noisy environments where adults are free to talk loudly and expressively. You must be able to work effectively and produce quality work under pressure. While driving to training’s or performing other duties there may be exposure to weather conditions prevalent at the time, noise, moving vehicles, high crime rate areas, odors, fumes, pollen etc.
Physical Requirements The Incumbent typically spends time sitting, standing, typing, thinking, writing, walking, driving, carrying
(max. 50 lbs), listening, speaking.
Mental Requirements The incumbent to be able to perform effectively under any and all of the following: unpredictable situations, crises, constant distractions, interruptions, uncontrollable changes in priorities/work schedules; be able to process information, think and conceptualize. You must be able to effectively deal with personal trauma that may be triggered.
